Community Health Centers (CHCs) are local, non-profit, community-owned healthcare centers that have been operating in communities across the nation since the 1960s. This national network of health centers provides high-quality affordable primary care. Also known as Federally Qualified Health Centers, they are located in areas where care is needed but scarce.
Sapphire Community Health is a fee for service healthcare facility. Patients are expected to bring their insurance information or form of payment at the time of service. Options for reduced fees are available if you qualify for the Sliding Fee Discount. Under certain circumstances payment plans may be arranged. Sapphire Community Health accepts cash, check or credit card.
As a Federally Qualified Community Health Center (FQHC), Sapphire Community Health provides care for patients with both Medicare and/or Medicaid benefits. Medicare patients’ deductibles are waived on office visits and on other FQHC services.
At Sapphire Community Health we provide a wide range of services including: immunizations, well child checks, men’s and women’s health physicals, management of acute and chronic diseases, mental health counseling, substance abuse counseling, pharmacy services, and more.

At Sapphire Community Health we welcome all, regardless of ability to pay. We are patient-centered and believe great health is about more than just healthcare. It’s about building a strong healthy community - one individual at a time.
Sapphire Community Health accepts most private insurance; as well as Medicaid, Medicare, Healthy Montana Kids, VA Healthcare, and health insurance assistance and tax credits through the federal Health Insurance Marketplace.
Fees are determined based on your ability to pay, taking into account income and family size.
Through a partnership with local pharmacies, Sapphire Community Health is able to provide medication assistance for eligible patients.
Most medications that are covered under this federal program are free or significantly reduced cost. The 340B Drug Discount Program is a Federal government program that requires drug manufacturers that participate in Medicaid to provide outpatient drugs to eligible health care organizations at significantly reduced prices.
You can bring in proof of income when you come for your visit. The best form of proof is a copy of your latest income tax return. Other types of proof include pay stubs, W-2s, Social Security documents, 1099s, etc. Remember that we need all household income for all adults who live in your household.

Sapphire Community Health welcomes all patients regardless of income. We accept most insurance, whether you are above or below the poverty line. While we don’t give discounts to those over the poverty level, we will still treat you. If you qualify for a discount and have insurance, the discount will be applied after your insurance pays, which can be especially helpful for those with high deductibles.

Yes! Sapphire Community Health provides physical exams and immunizations to people of all ages. We also offer seasonal flu shots to children and adults.
Family planning services, like the ones offered at Sapphire Community Health, often serve as the entry point for people seeking healthcare and are the only source of healthcare for millions of low-income individuals in the United States. Without family planning agencies, many women would not have access to yearly physicals, screening for cancer and sexually transmitted diseases, or other important services.
Additionally, family planning programs encourage teens to involve their families in their decision to seek family planning services. They also provide counseling to teens on how to avoid coercive attempts to get them to have sex.
Sapphire Community Health does not provide abortion services.
Sapphire Community Health is connected to a wide network of specialists throughout the state, and referrals can be made to any of these should your care require it.
